Nowadays,
for those who are missing teeth, regardless if they are missing
one, several, or all, tooth implants present a modern and
esthetic solution. We can replace one tooth, regardless,
if it's missing due to an accident, for hereditary reasons,
or even if it had to be extracted due to caries. In these
cases, a bridge is the conventional solution, but to avoid
preparing an intact adjacent tooth, implants are the best
option. The replacement of more than one tooth is indicated,
if tooth loss results from gingival recession or the extraction
of carious teeth. In this case, we have to install several,
independent implants, which serve as abutments for the future
bridge. In case of edentoulism, we still install several
independent implants in the jawbone, which serve as the base
for the prosthetic replacement. The result is structurally
more stable, more esthetic, and in every aspect superior
in quality to conventional dentures.
After tooth extraction, the surgical area has to ossify,
which takes approximately 6-8 months; only after this
time can we
install implants. The implants can be connected to the patients
natural tooth, or to another implant. Implants can only be
performed on patients 16 years or older, because development
of the jawbones has to be completed.
Preparation for surgery
The surgery is always preceded
by a dental examination, digital x-ray, and consultation.
Here we record the existing status of the teeth, and based
on this we make a treatment plan. To determine the precise
location of the implant, a panoramic x-ray, which allows
for a more comprehensive view of the mandible and maxilla,
is necessary. In every case, we take a study model of both
jaw bones. The model allows us to plan implant placement
and final restoration.We give a detailed treatment plan
presentation to the patient, which includes discussing
potential complications, as well as the possible esthetic
outcome.
The surgery
We highly recommend eating
before surgery, because after the injection of the anesthetic,
eating is not allowed for 3-4 hours, or until the anesthetic
wears off. We perform the procedure in local anesthesia,
which implies infiltration of the surgical area with 2%
Lidocaine. The dentist reflects the full thickness flap
from the bone, and by using a special bur, establishes
the depth and alignes the axis of the implant recipient
site. Into this recipient site, the dentist screws in a
titanium-based implant, which can be viewed as the root
of the new tooth. We close the surgical wound with sutures.
The screw placed in the bone will take 4-6 months to heal.
During this time, the space between the titanium screw
and the surrounding bone undergoes ossification, embedding
the implant into the bone. The screw cannot be loaded during
the healing phase. That's why a provisional bridge or denture
can be made during this time. The second operation takes
place when the screw is firmly embedded in the jawbone.
Next, we expose the screw from underneath the gum tissue
and place a small head on it, which elevates it above the
gum. We call this a healing cap. The gum will heal for
2-3 days, then the last phase of treatment takes place,
which does not require surgery. During this phase we attach
an abutment, which serves as the foundation for the future
bridge. Following this and the impression, the dental technician
will construct the appropriate restoration in multiple
phases.
Possible
complications
According to statistics,
the implants have a 95-99% success rate. Possible complications are extremely rare. Due to certain ossification problems,
some implants might not bond properly with the bone.As
with every oral surgery, certain complications can occur;
the patient can expect swelling, sensitivity and pain.
The success of the implants depend largely on how the patient
maintains the screw edges at the gingival margin. If the
bone of the implant becomes infected and inflamed, the
implant may become loose, or even fall out.
Postoperative care
The day after surgery, the
wound should be washed with a soft toothbrush. The patient
can rinse with Chlorhexamed 2-3 times a day. Compresses
are forbidden. Applying an ice pack to the exterior of
the face 5-6 times a day is allowed. Minor bleeding might
occur for a couple of hours following the procedure. If
necessary, medication for pain-relief is recommended. Antibiotics
should only be taken when instructed by the dentist. Sutures
will be removed 5-7 days following surgery. In case of
any problems, please call your dentist immediately.
