Food
remnants stick to the surface of the teeth. If they are not
constantly removed (by brushing), bacteria will multiply
in the dental plaque, and start breaking down the food remains.
The acids that the bacteria produce during the breakdown
process, will start to demineralize the enamel of the tooth.
The process spreads from the surface, across the enamel and
destroying the softer dentin layer might spread to the pulp.
Caries may develop on the tooth surface, approximally or
on the cervical surfaces. It may also develop near leaking
margins of restorations, inlays and crowns.
Caries is difficult to visually
identify during its initial stage, because the white, opaque
discoloration will only
occur on the enamel surface. Only a dental examination will
be able to identify it. At alater stage of the process, consumption
of sweets and inhalation of cold air will result in pain.
At this stage patients might also identify a black hole in
their tooth. In this case, the patient should immediately
consult their dentist, because with prompt treatment, the
tooth can be saved with a simple restoration. However, if
the tooth reacts with sharp pain to both hot and cold sensations,
causes discomfort, especially, during the night, or dull
pain during the entire day, then the problem can only be
solved by root canal treatment.
Preparation for treatment
A detailed dental examination
and digital x-ray of the diseased tooth and its surrounding
area precedes every treatment. During the following consultation
the treatment plan, type of anesthesia, type of materials
used, possible complications, and postoperative care will
be explained in detail. Eating is recommended before any
dental treatment, because the patient cannot eat for 3-4
hours after the administration of the anesthetic, until the
anesthetic wears off.
The inlay procedure
This type of restoration is
more precise, but since the dental technician requires an
impression for the restoration, it takes two appointments
to complete. Similar to regular restoration, we remove the
carious lesion of the tooth, after anesthesia, and take an
impression of the prepared cavity. Next, we place a temporary restoration in the tooth. At the next appointment, we replace
the temporary restoration with the one provided by the dental
technician. The inlay can be made of different materials.
Gold inlay
The best restoration made
by a dental technician. The main advantage of gold, is its
ability to be burnished onto teeth. We previously mentioned,
that even in restored teeth, caries can develop at the margins
of the filling. Since the edges of gold inlays are burnished to the margins of the tooth, there is no gap, thus giving
the restoration the longest life span. Gold does not discolor.
A disadvantage of gold is that many patients dislike the
esthetic effect, and also, because it is gold, it is more
expensive.

Gold-ceramic inlay
The advantage of this inlay
is its precise edges due to the polishable nature of gold,
and it is esthetically appealing, because the visible portion
is made from porcelain. Only the marginal edges are gold
colored; the occlusal surface is white and esthetic. No discolororation
occurs.

Resin inlay
Also requires an impression.
Its advantage over regular restorations is its more precise edges and a more esthetic occlusal surface. We use a special
cement for securing the inlay in the cavity. Tooth- colored
restorations, which are done chair-side, can shrink due to
the UV lamp's effect, and thus a virtual gap could develop
between the tooth and the restoration. Bacteria might penetrate
through this gap causing caries and further damaging the
tooth. With a resin inlay, we can also build up those teeth,
where a significant portion is missing. This is impossible
with tooth- colored chair-side restoration. It's disadvantage:
in 3-4 years it might be discolored by food dyes.

Post-inlay complications
Bite sensitivity, if the restored
tooth is too high. This can be adjusted by the dentist. Cold
or hot sensitivity could last for up to a week. Soreness around
the injection site diminishes in a couple of days.
