If
the caries is so severe, that it extends or nears the pulp
chamber, which contains vessels and nerves, pulpitis develops.
Another reason for root canal treatment, is necrotized
teeth due to an oversized restoration, or widespread caries. Since
the blood circulation in the tooth ceases, the blood is no
longer able to remove bacteria, which begin to multiply in
the periodontium and the surrounding bones - this can lead
to infection. Infected teeth can be responsible for inflammatory changes at other sites in the body (such as alopecia areata,
skin rashes, ophthalmological, urological or joint inflammations).
The pulp could also be infected if the tooth fractures, or
in case of severe gingival recession, when the bone does
not adequately cover the teeth. If we are preparing the tooth
for crowns on incisors and premolars, root canal treatment
is necessary in almost all cases, because we need to remove
significant amounts of tooth structure to assure space for
the future crown.
Symptoms that indicate the
need for root canal treatment
In non-severe cases, sharp
pain in reaction to sweets and cold, with immediate relief
when the stimulus is eliminated. Dull pain during the day,
which escalates or even becomes unbearable during the night,
when in a horizontal position. Strong throbbing pain or tension
around the teeth. Bite sensitivity. In severe cases, swelling
of the face.
Preparation for surgery
A detailed dental examination
and digital x-ray of the diseased tooth and its surrounding
area precedes every treatment. The following consultation
will explain in detail the treatment plan, type of anesthesia,
type of materials used, possible complications, and postoperative
care. Eating is recommended before any dental treatment,
because the patient cannot eat for 3-4 hours after the administration
of the anesthetic, as long as numbness persists.
The root canal treatment
Regardless of the reason for
root canal treatment, the procedure is always similar. After
the local anesthesia of the teeth (with a 2% Lidocaine injection),
the excess carious parts are removed with a bur. By cleaning
and shaping the canal with sterile files, we mechanically
remove the vessels and nerves that are located there. Throughout
the procedure we apply irrigation to the area, to chemically
reduce the number of bacteria. We slowly push the file deeper
into the canal, while continuously taking x-rays to determine
when the file reaches the apex. After all the canals have
been cleaned and shaped, they are irrigated and dried. Obturation of the canal is done with the newest materials; the goal is
to hermetically seal the canal. In most cases, the entire
procedure can be completed in one visit. If we are dealing
with a necrotic tooth, we inject medication into the root
canal for a week, then place a temporary filling into the
tooth, in order to reduce the number of bacteria. The final
obturation will be completed at another time. The price of
root canal therapy depends on the number of canals in the
tooth.
Post-obturation complications
Teeth which recently underwent
root canal treatment, can be sensitive to bite for a couple
of days. Since the circulation in root canal treated tooth
stops, they become dry and brittle. In order to preserve the
coronal structure, a crown is advisable on root canal treated
teeth.
