Large,
pendulous breasts that may be constitutional or develop for
other reasons, frequently cause physical complaints as well
(back and shoulder pain, irritation, fungal or bacterial
infections of intertriginous areas of the skin). Most frequently
following pregnancy and breast feeding, in other cases after
large weight loss, the skin and glandular tissue of the breast
enlarge, elongate and the breast becomes "pendulous".
A potential indication for surgery is the congenital or acquired
(due to surgery or injury) asymmetry of the breasts. Of course
these esthetic damages may lead to severe psychological problems
beyond the above mentioned physical symptoms.
Treatment options
The patient needs to openly
discuss his or her expectations with the surgeon, however
one has to keep in mind that the outcome to be expected is
improvement of the present condition and not achievement
of a perfect condition. Plastic surgery of the breasts changes
the size, shape, position, and tautness of the breasts. The
esthetical surgery will improve your appearance and self-confidence.
One day surgery
"One day surgery" means,
that patients can leave the outpatient center on their own
feet, accompanied by an escort, within 24 hours of surgery
(usually after few hours of observation), and continue the
healing process in the familiar confines of their own home. "One
day surgery" requires strict professional, social, and
various other preconditions that are discussed at the stage
of the consultation when the need for surgery becomes clear.
(i.e. good general health, absence of serious systemic disease,
patient must take responsibility for arrangement of transportation
and home care, must have phone to contact the clinic or the
surgeon if necessary, etc).
Preparation for surgery
The patient is evaluated by
the plastic surgeon prior to the procedure.
After the exam the surgeon discusses options with the patient
that might
affect his or her decisions with regard to the surgery (the
desired changes in form or shape). Then we inform the patient
whether he or she is a candidate for one day surgery. At
this point we also discuss one day surgery as a special treatment
option, the lab tests and other accessory studies (laboratory,
medical) that are necessary, types of surgical anesthesia,
possible complications, home care-related issues, phases
of healing, in one word, all the information that is necessary
for the patient to determine which treatment option to chose.
The procedure
The procedure involves correction
of the position, shape and size of the breasts. If size is
the main problem we remove the necessary skin and breast
tissue; however if the size of the breasts is different,
depending on the patients constitution and other factors,
reduction of the larger breast or augmentation of the smaller
with an implant may be performed. Correction of shape and
position of the breasts is accomplished by removal of the
excess lax skin and securing the breasts in their physiological
position. Following careful hemostasis the wound is closed
with cosmetic sutures. Frequently suction tubes are placed
into the surgical area to drain serous secretions. At the
end of the procedure dressing is applied to the chest.
Possible complications
No doctor can guarantee an
absolutely complication-free procedure. Complications may
occur despite maximum care. Early complications: serous accumulation
or hematoma, postoperative bleeding, inflammation, skin necrosis,
if recognized early are treatable. Observation of the patient
for 1-2 days postoperatively enables us to recognize early
complications in time. If at home, after leaving the clinic,
you notice the slightest sign of complications you should
call your surgeon immediately who will provide treatment
for the complication. Temporary paresthesias of the skin
or nipples may occur. One late complication is scar hypertrophy
or keloid, if it occurs it can be treated with injections
within 3 months after surgery. This procedure like other
surgical interventions carries risks, but is very commonly
performed in our clinic and our patients always leave satisfied.
Postoperative care
The patient is discharged
home after an observation period of 1-2 days. The suction
drains are removed prior to discharge. Antibiotics and if
needed analgesics are recommended after surgery. During the
first week at home light activity is recommended, lifting
of arms, frequent leaning should be avoided. The first follow-up
is scheduled for one week, sutures are removed after 2-3
weeks. Wearing an appropriate-sized sports bra is necessary
for 6 weeks.
