REMOVAL OF PIGMENTED NAEVI, FIBROMAS AND OTHER WARTS

 
The abnormality

The usually raised, variously pigmented, verrucous skin lesions may be pigmented naevi, warts, fibromas, teleangiectasias, etc. Generally, they are harmless, and their removal is for aesthetic reasons only; however, if the moles are at sites exposed to irritation, removal is appropriate from a medical perspective as well. Warts (Viral verrucas) are caused by viral infection due contact through micro-injuries of the epithelium in susceptive persons especially in childhood and young grown-up age. For the formation viruses belonging to family of HPV (Human Papilloma Virus) are responsible.

Examination and consultation

Surgery is always preceded by an examination by the surgeon. Following the examination, the doctor informs the patient about the results of the examination, outlines the suggested treatment plan, informs about anesthesia, as well as postoperative care. We will also show a photo of the expected esthetic result. The operation can be performed immediately after the examination and discussion; or it can be performed at a later time. Before the surgery, the patient has to sign a consent form authorizing the center to perform the surgery.

Removal of pigment naevi

The surgery is performed with local anesthesia, which means infiltration of the area immediately below the nevus with 1% Lidocaine. The nevus is removed using energy of radio-waves to vaporize cells along a wolfram loop. Due to the nature of the procedure, heat damage to the surrounding tissue is minimal, allowing to obtain a sharp-edged tissue sample suitable for histological exam. Removal of the pigmented nevus within healthy skin tissue in a transverse direction leaves an abraded area corresponding to the base of the nevus. This is covered with a band aid. The removed tissue is sent to pathology and the patient will be provided with a written report at the first follow-up visit.

In case of skin deformations with app. 1mm-2 cm scars remaining after radio wave operations carried out at our clinic have very nice esthetical result, but in case of larger skin deformations line-like scar remaining after plastic surgery cuts seems to be more ideal, therefore in such cases plastic surgery is indicated.

Possible complications


No doctor can guarantee a complication-free surgery. Complications can arise even following the most carefully performed procedures. The possible early complications: post-operative bleeding and inflammation, do not occur in our practice because the procedure involves only the surface of the skin. Among the possible later complications, scar hypertrophy (keloid) deserves mention. This is the consequence of a genetic predilection to abnormal wound healing and increased scar formation that is prominent, red and may be associated with itching and burning sensation. This is an essentially benign but esthetically bothersome lesion. If during follow-up visits we notice the tendency to this type of wound healing we can stop or reverse the abnormal process with injection therapy. This involves administration of long-acting anti inflammatory injections directly into the scar, three times, two weeks apart. Early recognition of abnormal wound healing is very important since it can only be successfully treated within three months of surgery. This is another reason in addition to learn about histology results to keep the follow-up appointment.

Postoperative care

The band aid should be removed after about two hours. The wound will dry and heel with crust formation like an abrasion. Protection from water and further dressing is not necessary. The crustshould fall off within 8-16 days, depending on the size of the wound, leaving a pink spot, which gradually fades during the next 2-3 months. The final result is a slightly lighter spot on the skin, corresponding to the base of the nevus that is level with the skin. The first follow-up examination is six weeks after the surgery. This is when the patient receives the histology exam results.
