A
full acrylic denture is only fabricated
when the patient is missing
all of the original teeth. This not only restores the chewing
function, but has an esthetic effect as well. Partial denture
is fabricated when the patient still possesses some teeth,
however, not enough in numbers or not
properly positioned
for a bridge. If molar teeth are missing, a partial denture
becomes necessary to restore the chewing function.
Denture types, materials
A complete acrylic denture
is made for edentulous patients. The partial denture is cast
metal, covered by acrylic. In both cases the teeth are acrylic.
Porcelain teeth cannot be used in the denture, because their
weight would pull the denture from its place. Partial dentures
can be connected to existing teeth, crowns, and bridges in
a variety of ways. The easiest method is to employ metal
clasps. However, this is esthetically not the optimal solution,
because the arm of the clasps is visible at the gum line.
Esthetically, the best solution is achieved by using precision
attachments. In this case, the bridge and the removable denture
have to be fabricated at the same time. This is called combination
work, because there is a fixed as well as a removable component.
One part of the attachment is connected to the bridge, the
other part to the denture. When the denture is snapped in
place, the attachment is not apparent. Another advantage
to this method is the stability of the denture.
Making the denture
Fabrication of bridges for
combination works is explained in the crowns and bridges
section. All types of dentures are made in a similar way.
During the first visit, we take preliminary
impressions of both the maxillary and mandibular arches,
which allows the
technician to make a custom tray. This helps us attain a
more precise impression and fabrication of a more stable
denture. During this visit, we also measure the central occlusion.
This is important, because we are able to fill the void left
by the missing teeth with a wax rim, which supports both
the face and the lip, eliminating the wrinkles associated
with a caved in mouth. During the next phase, we try in the
teeth in wax. At this time we are still able to modify the
size, shape and position of the teeth according to the patients
wishes. Adjustment of the teeth in wax, requires another
try in. Finally, when all teeth are esthetically correct,
the dental technician completes the final denture. The whole
process takes about two weeks form the first impression,
or three weeks in case of combination work.
Post-denture complications
Like a new shoe, a new denture
takes getting accustomed to. Sensitive areas of the gum might
become sore. This can be adjusted at the office. The patient
needs to wear the denture for half a day before the adjustment,
because this amount of time is needed for the sore spots
to become visible to the dentist. The facial muscles also
need to get accustomed to the new denture; talking and chewing
might be strange initially. If during chewing, there is premature
contact between the teeth, adjustments can also be made.
If the denture is not cleaned properly, bacteria and fungi
can spread, which could initiate various inflammatory reactions.
Therefore, it is recommended, that in addition to using denture
cleaning tablets at night, the denture should be cleaned
after eating as well. Dentures cause continuous bone and
gum recession, therefore, a new denture should be fabricated
minimum every eight years to accommodate the changing conditions
in the mouth.
Warranty
The technician guarantees
the denture for a year. The interim denture is not included
in the warranty due to its temporary nature. The warranty
is only valid, if the patient appears at the half-year follow
up examinations.
