Inflammation
of Bartholin glands is Inflammation of Bartholin glands is
caused by pyogenic bacteria in the draining ducts of the
glands. Swelling leads to ductal obstruction, preventing
the drainage of glandular secretions. Trapped secretions
may become infected leading to abscess formation within 1-2
days. Symptoms include gradually increasing pain, swelling
in the lower third of the inner and outer lips on one side
of the vaginal opening, difficulty sitting and walking, slight
fever, or even high temperature.
Three forms of this condition are known:
- Bartholin abscess
- Chronic recurrent inflammation
of the Bartholin glands
- Bartholin cyst
Treatment options
All three forms require surgical treatment
for permanent resolution. Surgery is generally performed
on one day surgery basis.
Examination and consultation
The gynecological exam includes
inspection and careful palpation of the Bartholin glands.
Pelvic, colposcopic and cytological exams are performed only
after resolution of the inflammation. Pre-surgery labs include
complete blood count, coagulation studies and microscopic
and microbiological examination of urethral secretions.
Fee of examination and consultation: 10.000 HUF > Euro
One day surgery
"One day surgery" means,
that patients can leave the outpatient center on their own
feet, accompanied by a helper, within 24 hours of
surgery (usually after few hours of observation), and continue
the healing process in the familiar confines of their own
home. "One day surgery" requires strict professional,
social, and various other preconditions that are discussed
at the stage of the consultation when the need for surgery
becomes clear. (I.e.: good general health, absence of serious
systemic disease, patient must take responsibility for arrangement
of transportation, home care, and must have phone to contact
the clinic or the surgeon if necessary, etc).
Preparation for surgery
At the booked operative time patients must bring their lab tests, slippers, night-suit. Before placed into sick-room a "STATEMENT" is to be signed in order to entitle our clinic for the performance of the procedure. Besides patients have to undertake home wound care. Important!
Considering that operation is performed in general anesthesia, food and drink intake is allowed until 24:00 pm on the prior day to procedure -for patient have to arrive with empty stomach for the surgery!
Please take the regularly taken drugs in time and dose. Take morning drugs with little liquid at home before departure and in case you may have drugs to be taken in day-time, please bring them with to the clinic!
Surgery
After signing the consent
form, the patient changes clothes and proceeds to surgery.
The surgery is usually performed with local
anesthesia, only
large abscesses require general anesthesia. For Bartholin
abscesses we perform incision and drainage, followed by placement
of gauze pads in the abscess cavity. Pads are changed daily
for 4-5 days following surgery. On postoperative days 3 to
5 sitz baths may be started. Antibiotics are generally not
needed. In case of relapsing inflammation or cyst formation,
we perform a marsupialization. This involves a 1-2 cm incision
at the skin-mucosal border, and suturing the inner edge of
the incision to the external mucosa to prevent the closure
of the wound.
Possible complications
No doctor can guarantee a
complication-free surgery. Complications can arise even following
the most carefully performed procedures. Possible early
complications like postoperative bleeding and inflammation if recognized
early are treatable with specific measures. If you experience
even the slightest indication of complications (for example
bleeding, fever or pain), you can contact your surgeon day
or night, who will take the necessary steps to resolve the
problem.
Postoperative care Surgery
is commonly followed by 6-8 hours of observation,
during which time the vital functions stabilize (blood pressure,
urine output), the sedative effects of premedications wear
off, and patients are able to leave the center on their own
feet, with the help of an assistant. The day following
surgery, most of our patients can continue their daily routine,
taking days off work is not necessary. The average time for
recovery after Bartholin gland surgery is 2-3 weeks.
During this time, use of tampons or sexual
activity is not recommended. In uncomplicated cases
we ask our patients back for follow-up four weeks after surgery.
