SURGERY FOR INGROWN TOENAIL WITH ONYCHOPLASTRY

 
The illness

In the majority of cases the great toe is affected: the usually constitutionally wider nail due to some external cause (incorrect trimming, tight shoes) is squeezed on one or both sides against the surrounding tissues leading to an inflammatory response. This is called the "ingrown toenail" symptom complex: pain, redness, swelling, discharge, inflammatory granulation tissue. The condition always affects both sides of the toe, even if symptoms do not occur on both sides at the same time. We can perform surgery on only the affected side, however, the un-operated side will eventually exhibit similar symptoms, and the procedure will have to be repeated. The second option is to perform surgery on both sides, in which case we completely solve the problem with one surgery and one healing period. The choice is completely with the patient, professionally both are acceptable.

Examination and consultation

Surgery is always preceded by evaluation by the surgeon. Next, we inform the patient about the surgery, anesthesia, home care, and possible complications. The surgery can be performed immediately following the examination and discussion, or at a later date. If the patient decides on surgery, a "DECLARATION" has to be signed, which contracts the clinic to perform the surgery. The patient also has to take responsibility for wound care at home.

The procedure

Surgery involves, after local anesthesia, the excision of the ingrown nail edge and narrowing the nail to the desired width by destruction of the lateral nail producing cells, using laser. Thereby we can set the width of the nail such that it will not push against the surrounding tissues. This procedure results in complete recovery.

Possible complications

No doctor can guarantee a complication-free surgery. Complications can arise even following the most carefully performed procedures. Possible early complications: post-operative bleeding and inflammation, are easily treatable, therefore as soon as symptoms appear patients should contact their surgeon. On the day following surgery, after soaking for five minutes in warm water, the patient should unwrap the dressing and remove the tampon placed by us with a quick resolute movement. Minor bleeding might occur and should not cause concern. The wound should be rinsed thoroughly, patted dry and dressed; this will tamponade the bleeding. Inflammatory complications can only develop, if the twice a day soakings are neglected, because if the wound becomes dry the accumulating secretions can not drain. If this occurs the wound has to be soaked thoroughly and then by applying firm pressure to the sides secretions should squeezed out and the inflammation will resolve within 24 hours. Later complications include recurrence of ingrown toenail if for some reason the nail-producing cells were not entirely eliminated. According to our statistics this happens only in 4-5% of the cases. In such cases we can of course repeat the surgery under warranty, without the patient having to pay additional surgical fees.

Postoperative care

The day after ingrown toenail surgery, the dressing and tampon need to be removed. The wound should be rinsed with warm water twice a day, patted dry and covered with band aid. Discharge fromhealing wounds after laser surgery is normal. Reddening of the wound boarder is also a part of the normal healing process, and not indication of inflammation. After a week at home it is practical to leave the wound open, this will expedite the healing process. Powders and creams should not be applied to the wound. Complete recovery is between 4-6 weeks depending on the pre-surgery condition of the nail. Please keep your follow-up appointments, failure to appear will void the guarantee.
