Varicosity
is dilatation of the superficial veins of the lower extremity.
Genetic factors (i.e. weakness of the vessel wall) also play
a role in the development of the condition. The basic cause
that leads to the development of the illness is defective
valves in the veins that connect the superficial and deep
venous systems. Normally these valves allow blood flow only
towards the deep veins, but if for some reason they do not
close perfectly the pressure in the deep venous system is
transmitted to the thinner-walled superficial veins gradually
dilating them. In the dilated veins the blood flow slows
even stops (especially when standing in one place). This
congestion is responsible for the symptoms associated with
varicose veins: pain, pressure, fatigue. The dilated veins
are prone to the development of thrombosis and inflammation,
later, and poor circulation may even result in the development
of ulcers on the extremity.
Treatment options
In early stages of
varicosity use of compression
stockings of various degrees may be considered.
In case of advanced varicosities only surgery offers complete
cure, this has to follow an evaluation by the vascular surgeon.
A prerequisite for surgery is an intact deep venous system
that can be confirmed by appropriate studies. Timely surgery
has very good outcome. The average time for recovery is about
3-4 weeks, this means that the patient can resume normal
work by this time. The only 2-3 mm incisions give nice esthetic
result leaving only minute scars.
One day surgery
"One
day surgery" means, that patients
can leave the outpatient center on their
own feet, accompanied by a helper, within
24 hours of surgery (usually after few
hours of observation), and continue
the healing process in the familiar
confines of their own home. "One
day surgery" requires strict professional,
social, and various other preconditions that are discussed at the stage of the
consultation when the need for surgery
becomes clear. (I.e.: good general health,
absence of serious systemic disease,
patient must take responsibility for
arrangement of transportation, home
care, must have phone to contact the
clinic or the surgeon if necessary,
etc).
Examination and consultation
The
procedure is preceded by evaluation of the surgeon. We evaluate the health
of the deep venous system (if damaged,
surgery affecting the superficial venous
system can not be performed) and the
condition of the largest valves between
the two venous systems. These will determine
the surgical procedure of choice. If
the valve in the main branch is intact
only the dilated veins need to be removed.
If however the valve is damaged the
main venous branch along its entire
length hast to be removed as well. After
this we inform the patient whether from
a surgical standpoint he or she is candidate
for one day surgery. It is at this point
that the patient is acquainted with
the concept of one day surgery. It is
also here, that the patient is informed
about laboratory and other accessory
studies (Imaging, ECG, medical) that
might be required for surgery, types
of surgical anesthesia, possible complications,
home care-related issues, stages of
recuperation, possoble problems during
recovery i.e. all the information that
is relevant for the patient to make
a decision on which treatment method,
or surgery to choose.

Preparation for surgery
At the booked operative time patients must bring their lab tests, slippers, night-suit. Before placed into sick-room a "STATEMENT" is to be signed in order to entitle our clinic for the performance of the procedure. Besides patients have to undertake home wound care. Important!
Considering that operation is performed in general anesthesia, food and drink intake is allowed until 24:00 pm on the prior day to procedure -for patient have to arrive with empty stomach for the surgery!
Please take the regularly taken drugs in time and dose. Take morning drugs with little liquid at home before departure and in case you may have drugs to be taken in day-time, please bring them with to the clinic!
The procedure
On the
day of the surgery, the patient signs the
consent form, then the necessary lab tests
are done and the surgery fee is paid. Immediately
before the operation, the patient is premedicated with an injection containing analgesic
and sedative. The operation can be performed
under local, spinal or general
anesthesia.
The majority of our surgeries are performed
with local
anesthesia, which means infiltration
of the surgical field with 1% Lidocain.During
the procedure, if the main venous branch
has to be removed it is opened from 3 cm
incisions made at the groin and the ankle,
a plastic cannula is inserted into the
vein and it is stripped out. The dilated
accessory branches are removed one by one
through 2-3 mm incisions. Finally the extremity
is wrapped with elastic dressing.
The duration
of surgery is 15-25 minutes.
Possible complications
No doctor can guarantee a
complication-free surgery. Complications can arise even following
the most carefully performed procedures. The possible early
complications: post-operative bleeding, inflammation are
easily treatable.Postoperative bleeding is easily to recognized,
as well as inflammation by pain, redness and warmth. Therefore
f you experience even the slightest indication of complications
(i.e. bleeding or fever), you can contact your surgeon day
or night, who will take the necessary steps to remedy any
of the complications. As the healing progresses various changes
may occur that are not always abnormal. Such are swelling
and bruise on the affected extremity these disappear in a
few weeks. A possible late complication is recurrence of
varicose veins. This of course does not mean the reappearance
of the removed veins but dilation of veins at other sites.
The role of correct surgical technique is to make late complications
a remote possibility.
Postoperative care
The surgery
is commonly followed by 6-8 hours of observation,
during which time the vital functions (blood pressure, urine
output)
stabilize, the sedative effects of the premedications wear
off, and patients are able to leave the center on their own
feet, with the help of an escort. During the first week
the patient should refrain from physical strain, however
constant bed
rest is not necessary. Sutures are removed at the one week
follow-up. Full recovery takes 4 weeks, by this time the
hematomas formed during surgery are resorbed. During this
time only light physical activity is recommended. The second
control examination takes place after four weeks, at this
time we
find healed
status.
