The
nose is one of the most conspicuous parts of the face, its
deformities may cause severe psychological problems. Nose
deformities may be congenital, or acquired (i.e. post trauma).
The difference may be in size (too big, too small,) shape (hooked, pug, wide, crooked, etc.), anatomy (deviated nasal
septum, narrow, deformed nostrils) and may cause esthetic,
or functional complaints(i.e. breathing difficulty).
Treatment options
Most of the problems can be
corrected surgically. The patient needs to openly discuss
his or her expectations with the surgeon, however one has
to keep in mind that the outcome to be expected is improvement of shape and contour and never the achievement of a perfectly
ideal condition. The surgery will enhance your appearance
and self-confidence and if impaired nose-breathing is present
will solve that as well.
One day surgery
"One day surgery" means,
that patients can leave the outpatient center on their own
feet, accompanied by an escort, within 24 hours of surgery
(usually after few hours of observation), and continue the
healing process in the familiar confines of their own home. "One
day surgery" requires strict professional, social, and
various other preconditions that are discussed at the stage
of the consultation when the need for surgery becomes clear.
(i.e. good general health, absence of serious systemic disease,
patient must take responsibility for arrangement of transportation
and home care, must have phone to contact the clinic or the
surgeon if necessary, etc).
Preparation for surgery
The patient is evaluated by
the plastic surgeon prior to the procedure. After the exam
the surgeon discusses options with the patient that might
affect his or her decisions with regard to the surgery (the
desired changes in form or shape). Then we inform the patient
whether he or she is a candidate for one day surgery. At
this point we also discuss one day surgery as a special treatment
option, the lab tests and other accessory studies (ENT and
medical consultations) that are necessary, types of surgical
anesthesia, possible complications, home care-related issues,
phases of healing, in one word, all the information that
is necessary for the patient to determine which treatment
option to chose.
The procedure
Rhinoplasty is usually performed
under general anesthesia, or if the deformity is minor local
anesthesia may be used. The procedure involves sculpting
the bones and cartilage of the nose to the desired
shape and securing the nose in the new position. Surgical incision
is made inside the nose thus the scar after healing is not
visible from the exterior. An exception to this is the procedure
for reduction of the nostrils where a skin excision made
at the site of attachment will appear as a thin linear scar
after surgery. If deviation of the nasal septum is present
this will be corrected first by an ENT surgeon, then are
deformities of the visible areas of the nose corrected by
the plastic surgeon. Rhinoplasties complicated by nasal septum
are usually operated simultaneously, but may also be performed
in two separate sessions. Following rhinoplasty, after surgery
nasal packs are placed into the nostrils, and a plaster splint
is applied to the nose externally.
Possible complications
No doctor can guarantee a
complication-free procedure. Complications may occur despite
maximum care. Early complications: postoperative bleeding,
inflammation if recognized early are easy to treat. The purpose
of the 4-6 hour postoperative observation is recognition
of early complications. If at home, after leaving the clinic,
you notice the slightest sign of complications you should
call your surgeon immediately who will provide treatment
for the complication. Late complications: necrosis of the
skin or cartilage, temporary sensory deficit, breathing difficulty,
abnormal scar formation (keloid). The latter can be treated
with injections within 3 months after surgery. Other, esthetic
complications: irregular surface contour, asymmetry, change
in skin color may be avoided with correct surgical technique.
This procedure like other surgical interventions carries
risks, but is very commonly performed in our clinic and our
patients always leave satisfied.
Postoperative care
Following this procedure at
least 16-18 hour observation is necessary. During this time
vital functions (blood pressure, urine, stool) stabilize.
Since the procedure involves a relatively large area taking
antibiotics and analgesics is recommended. Nasal packs placed
during surgery are removed the next day and the patient is
discharged home accompanied by an escort. Plaster splints
need to be worn for one week. Local pain can be expected
for 2-3 days and may be controlled with usual oral analgesics.
For a few days after removal of nasal packs serosanguinolent
fluid may drain from the nares, that can only be wiped off.
Blowing the nose or leaning forward should be avoided since
they might cause postoperative bleeding. Swelling of the
surgical area around the nose or eyelids may cause transient
annoyance by interfering with breathing or vision. Hematomas
formed on the face are entirely harmless and are usually
resorbed within 8-10 days. Plaster splints are removed during
the first follow-up at one week. Following removal of the
splints special attention should be paid to avoiding injuries
to the nose since these may result in new deformities. Following
splint removal the swelling around the nose gradually diminishes
but the final results may not be apparent for weeks or even
months.
