Almost
everyone has a more or less bent nasal septum.
This natural phenomenon is a consequence of anatomical relations,
and the age-related relative difference in the rate of bony
and cartilaginous growth. In a smaller percentage of individuals
injury or developmental abnormalities may cause deviation
of the nasal septum. Despite septum deviations, the majority
of people can breathe normally, and they don't have other
septum related problems. Around 20-25 percent of the adult
population may be affected by very diverse symptoms related
to deviation of the nasal septum.
A list of the most important symptoms-without
claiming to be complete- that might prompt you to seek medical
attention:
- congested nose, either one side or both, in the absence
of cold symptoms
- smelling difficulty
- snoring
- frequent sinus headaches
- protracted or relapsing cold or sinus symptoms
- nasal voice
- bad breath, or dryness of the throat in the morning
- frequent unpleasant nasal or oro-pharyngeal discharge
(To avoid misunderstandings it needs to be stressed that
the above symptoms may have other causes as well)
Treatment options
Surgery is
the only long term, permanent solution. Since not all septum
defects cause problems, indication for surgery must be made
with great consideration. In children 16-18 years of age,
due to still ongoing bone growth, surgery should be performed
only in very severe, special cases.
One day surgery
"One day surgery" means,
that patients can leave the outpatient center on their own
feet, accompanied by a helper, within 24 hours of
surgery (usually after few hours of observation), and continue
the healing process in the familiar confines of their own
home. "One day surgery" requires strict professional,
social, and various other preconditions that are discussed
at the stage of the consultation when the need for surgery
becomes clear. (I.e.: good general health, absence of serious
systemic disease, patient must take responsibility for arrangement
of transportation, home care, must have phone to contact
the clinic or the surgeon if necessary, etc).
One day surgery
The surgery is always preceded
by an ENT examination. The decision to operate
will be based on the physical examination, possibly an endoscopic
examination, x-rays, and lab tests depending on whether the
patient wants to undergo surgery under local
anesthesia or general anesthesia. Following
these studies, the doctor will give a detailed explanation
about the severity of the condition and the possible treatment
methods. It is at this point that the patient is acquainted
with the concept of one day surgery. It is also here, that
the patient is informed about laboratory and other accessory
studies (Imaging, ECG, medical exam etc.) that might be required
for surgery, options for anesthesia, possible complications,
home care-related issues, stages of recuperation, i.e. all
the information that is relevant for the patient to make
a decision on which treatment method, or surgery to choose.
The septum surgery
Septum defects can manifest
in formations of different shapes and sizes, tremendously
complicating the job of the surgeon.
Septoplasty of the deviated
septum appears to be a good solution in almost
all instances. During this procedure, through an anterior
incision in one nasal cavity, not visible from the exterior,
the deviated septal cartilage is straightened out and rebuilt
while only the redundant parts are removed. Even the visible,
anterior, so called luxated nare can be made symmetric.
The mechanics, the back and tip of the nose do not change.
The surgery lasts approximately 1 hour.
In the rare instance when
the narrowing is caused by posterior septal deviation we
perform partial removal of the deviated septum.
In this case the incision is made much deeper in the nasal
cavity and the entire deviated septum behind the incision
is removed. The surgery lasts 30-40 minutes.
In either case, pads are
placed in both nostrils after the surgery that are removed
on the second post-operative day. Unfortunately, during these
2 days, the patient will not be able to breathe through the
nose.
Possible Complications
No doctor can guarantee a
complication-free surgery.Even with the most careful surgical
technique post-operative bleeding after removal of the pads,
infection, or penetration of blood between cartilaginous
membranes can occur. However, even if these early
complications arise, they are easily treatable.
For this reason, if you experience even the slightest indication
of complications (for example, bleeding or fever), you can
contact your surgeon day or night, who will take the necessary
steps to remedy any of the complications.
During the course of home
recuperation, patients might experience various changes,
however, these don't always prove to be abnormal. Aching
of the wound or fever during the days following surgery should
not be cause for concern. In some instances, a bizarre feeling,
tingling, numbness or possibly pain in the back of the nose,
on the hard palate or in the incisors might last for a couple
of weeks. Later complications (i.e. recurrence of the illness)
can be prevented by careful surgical technique.
Postoperative care
When breathing through the
mouth the oral cavity dries out rapidly, therefore it is
advisable to drink plenty of liquids and if possible use
a humidifier in the room of the patient. Carbonated drinks,
or spicy foods should be avoided. Patients should refrain
from all types of strenuous physical activities, such as
bending, lifting, or sports because these may increase the
blood pressure leading to post-operative bleeding and septal
hematoma. For the same reason, hot baths in tubs are to be
avoided, instead, quick showers should be taken.
Following removal of nasal
pads, minor bleeding may occur. Passage of blood clots or
thick mucus from the nose for a few days is normal.Patients
should not blow their noses hard. For one week nasal ointment
or oil should be applied regularly to aid mucosal regeneration.
Recuperation time is approximately
2 weeks. Sport activities should be resumed no sooner than
3 weeks following surgery.
