LIPOSUCTION

The condition

Esthetically unpleasant excess fat deposition in certain localized areas of de body, that persists stubbornly despite decreased calorie intake and exercise.

Treatment options

The patient needs to openly discuss his or her expectations with the surgeon, however one has to keep in mind that the outcome to be expected is improvement of body contours and not achievement of a perfect condition. Liposuction will change the contour of your body, enhance your appearance, establish your self-confidence. However it can not substitute for weight loss that can be achieved with diet and exercise. The procedure can be performed only in very carefully selected patients. Best candidates are patients with young elastic skin, relatively normal weight and local fat deposits.

One day surgery

"One day surgery" means, that patients can leave the outpatient center on their own feet, accompanied by an escort, within 24 hours of surgery (usually after few hours of observation), and continue the healing process in the familiar confines of their own home. "One day surgery" requires strict professional, social, and various other preconditions that are discussed at the stage of the consultation when the need for surgery becomes clear. (i.e. good general health, absence of serious systemic disease, patient must take responsibility for arrangement of transportation and home care, must have phone to contact the clinic or the surgeon if necessary, etc).

Preparation for surgery

The patient is evaluated by the plastic surgeon prior to the procedure. After the exam the surgeon discusses options with the patient that might affect his or her decisions with regard to the surgery (i.e. the site and amount of the fat to be removed). Then we inform the patient whether he or she is a candidate for one day surgery. At this point we also discuss one day surgery as a special treatment option, the lab tests and other accessory studies (i.e. imaging, ECG, abdominal ultrasound, medical) that are necessary, types of surgical anesthesia, possible complications, home care-related issues, phases of healing, in one word, all the information that is necessary for the patient to determine which treatment option to chose.

The procedure

Prior to surgery the surgeon exactly marks the sites for liposuction. This is followed by local anesthesia or general anesthesia. This is followed by injections of a medicated solution into the previously marked areas that serves to dissolve and loosen up the fat. The surgical intervention starts from one or more small, approximately 4-5 mm incisions on the skin. Through these small openings suction cannula is introduced under the skin and with the help of vacuum the excess fat is suctioned off. The procedure is repeated in all the marked areas, then the entry openings are closed with fine sutures. At the end of the surgery pressure dressing and a snug compression garment is applied.

Possible complications

No doctor can guarantee a complication-free procedure. Complications may occur despite maximum care. Possible early complications: edema, hematoma, postoperative bleeding, significant blood loss, inflammation, injury of vessels or nerves, pulmonary embolism, fat embolism, if recognized early are easy to treat. The purpose of the 4-6 hour postoperative observation is recognition of early complications. If at home, after leaving the clinic, you notice the slightest sign of complications you should call your surgeon immediately who will provide treatment for the complication. A late complication is excessive scaring or keloid formation. This can be resolved with injection treatment within 3 months after surgery. Esthetic complications: uneven surface contour, asymmetry, change in skin color may be avoided with correct surgical technique. This procedure like other surgical interventions carries risks, but is very commonly performed in our clinic and our patients always leave satisfied.

Postoperative care

The procedure is usually followed by a period of observation for 6-8 hours. During this time the vital functions (blood pressure, urine) stabilize, effect of premedications wear off and the patients can leave the clinic on their own feet accompanied by an escort. After spinal or general anesthesia the observation period increases to 16-18 hours. Analgesics are needed for a few days after surgery. Strenuous physical activity needs to be avoided during the first week but continuous bed rest is not necessary. At the one week follow-up the sutures are removed. Patients need to wear the compression garment for six weeks to prevent local edema and bleeding as well as for enabling the skin to better fit the new contours. Hematomas will be resorbed without trace in a few weeks. The skin may remain numb for a few days.

Strenuous work should be avoided in the first weeks. The scar tissue that starts to form from the second week does not allow the deposition of fat. Thus if later there is weight gain fat will be deposited in other areas.
