INFORMATION ON REMOVAL OF SCARS AND SKIN LESIONS

 
The deformation

Scars on skin can be lesions resulting due to operations, injuries. These in most cases cause esthetical problems, but for example those shrinking scars formed due to burnt - above joints can even limit movement. Scar plastic surgery can heal these problems.

Skin lesions generally raise above the skin level can be melanocytic nervi, "old-age" moles, port-wine stains etc. Majority of these are benign lesions, and are removed for esthetical reasons, but removal of irritated melanocytic nervi is medically indicated.

Treatment opportunities

Revision of scars resulting from operations or injuries is recommended the earliest one year following the formation. Those scars are exceptions that cause serious restriction on movement.

In case of skin lesions with app. 1mm-2 cm scars remaining after radio wave operations carried out at our clinic have very nice esthetical result, but in case of larger skin deformations line-like scar remaining after plastic surgery cuts seems to be more ideal, therefore in such cases plastic surgery is indicated.

Examination and consultation

The patient is evaluated by the plastic surgeon prior to the procedure. After the exam the surgeon discusses options with the patient that might affect his or her decisions with regard to the surgery. Then we inform the patient whether he or she is a candidate for one day surgery - if we can only perform the operation in general anaesthesia. At this point we also discuss one day surgery as a special treatment option, the lab tests that are necessary, types of surgical anesthesia, possible complications, requirements for home care, phases of healing, possible problems during the healing process, and anything else that is necessary for the patient to determine which treatment option to chose.

One day surgery

Due to size of deformations the operation generally performed with local anaesthesia, but mainly in case of scars with larger extents like wound plastics the intervention is carried out in general anaethesia. In local anaesthesia the operative area is injected with 1% Lidocaine solution. "One day surgery" means, that patients can leave the outpatient center on their own feet, accompanied by a helper, within 24 hours of surgery (usually after few hours of observation), and continue the healing process in the familiar confines of their own home. "One day surgery" requires strict professional, social, and various other preconditions that are discussed at the stage of the consultation when the need for surgery becomes clear. (I.e.: good general health, absence of serious systemic disease, patient must take responsibility for arrangement of transportation, home care, and must have phone to contact the clinic or the surgeon if necessary, etc).

Preparation for surgery

The patient is evaluated by the plastic surgeon prior to the procedure. After the exam the surgeon discusses options with the patient that might affect his or her decisions with regard to the surgery (the desired change in size or shape, etc.). Then we inform the patient whether he or she is a candidate for one day surgery. At this point we also discuss one day surgery as a special treatment option, the lab tests and other accessory studies (i.e. laboratory, medical) that are necessary, types of surgical anesthesia,the size, shape, type of the implant to be inserted, possible complications, home care-related issues, phases of healing, in one word, all the information that is necessary for the patient to determine which treatment option to chose.

The operation

During the operation the lesion is cut out via a bay-leaf-like cut, margins of a wound is prepared, than following to the careful haemostasis the wound is closed up by glover's suture or fine stitches. In case of lesions with larger extent if due to cut a huge skin-lack may occur, tissue dilating balloon may be needed. In this event at the first operation through the cut at the edge of lesion a tissue dilating balloon is placed under the skin. The balloon is filled up with physiological salt solution what is gradually repeated usually once a week. The balloon stretches the skin above continuously by its growth. Once the skin is stretched to enough extent, at the second operation the cut of skin lesion or scar is performed and the wound is finally closed up tension-free by the previously stretched skin.

Possible complications

No doctor can guarantee a complication-free procedure. Complications may occur despite maximum care. Early complications: oedema, haematosis, postoperative bleeding, inflammation, wound separation if recognized early are easy to treat. Observation of the patient postoperatively enables us to recognize early complications in time. If at home, after leaving the clinic, you notice the slightest sign of complications you should call your surgeon immediately who will provide treatment for the complication. Late complications are wound healing problems, sensing disorder, inflammation, wound separation, secondary wound healing, scar hypertrophy or keloid. If it occurs it can be treated with injections within 3 months after surgery.

Tasks after the operation

Patients can leave the clinic in case of scar plastic surgery carried out in local anaesthesia 2-3 hours, in case narcosis 10-12 hours after the operation with accompany. During this period of time physiological functions (blood-pressure, urine) get stable again. Patients have to take the organisation of their home care. At home it is very advisory to live carefully and take rests. If the scar plastic surgery affects relatively large area, taking antibiotics and pain-killers is advisory. On surgical area swelling and bluish-violaceous discolouration may appear what is considered a normal post operative state. With the use of a couple of days long ice-chilling these symptoms can be diminished, haematomas will slowly assimilate. The first control examination takes place one week after the operation. At this time the pressure bandage is removed. Stitch removal takes place two or three weeks after the operation. In the following period on scars for approx. one week Contratubex, Dermatix, silicone plaster, Bioptron lamp can be used in order to reach better esthetical result.

General anesthesia: 80.000 HUF > Euro
One overnight stay: 30.000 HUF > Euro

50% of the total cost has to be paid as a deposit before surgery.
The plastic surgeon defines charge of operation always after examination, considering that all deformations are unique.
